The Challenge
ACPN needed a clear, outcomes-focused framework to guide their programs and communicate impact to funders. Activities were happening in silos with no unified program logic, making it difficult to demonstrate results or differentiate support for home-based vs. center-based providers.
The Approach
We designed a collaborative process to build a revised Theory of Change and two aligned Logic Models — one for home-based providers and one for center-based providers. This included stakeholder engagement, document review, data integration, and differentiated 18-month outcome timelines.
